Are you in need of a chemical dependency assessment? I complete 245G compliant comprehensive assessments to provide appropriate treatment recommendations.

I have over three years of experience completing assessments for individuals requiring evaluation of their substance use due to court, probation, work, or the internal desire to seek a sober lifestyle.

My assessments are provided over the weekend to accommodate working professionals that otherwise have difficulty scheduling around their busy lifestyles.

Assessments are completed over telehealth.

Sword in Stone Counseling is a private-pay practice and does not bill insurance. Payment for services is due at the time of the appointment. If your insurance plan includes out-of-network benefits, I can provide a superbill that you may submit to your insurance provider for possible reimbursement. Reimbursement rates vary by plan.
